Irish Citizens who are over 18, and have previously held an Irish Passport can now apply for their Passport and/or Passport Card online at https://www.dfa.ie/passportonline.

If you are resident in the Czech Republic or Ukraine, you need to use an APS2E application form.
APS2 forms are available:
Irish citizens who are over 18, and currently hold an Irish Passport can now apply online for their new passport. (If you have already applied through Passport Express or your Irish Embassy/Consulate you cannot apply online as this will delay both applications).
Children, first time and name change applicants cannot apply online.
If the birth/marriage certificates are not in English, you must submit a certified English version of the original, translated by a registered translator.

Infants or very young children who can't support themselves should be photographed lying down on a plain, white surface. No one else should appear in the photo, so make sure that hands or arms used to support the child are not visible.

Witnessing in the Czech Republic
Identification (Section 9 of your application form)

A member of one of the professions listed below can witness your passport application form as long as they are satisfied as to your identify and that your four photos are a true likeness of you. Make sure you bring photo identification with you and be aware that the witness may ask you for more proof of identification - this is at their discretion.
Your application form is only valid for six months from the date your witness signs Section 9.
This is an additional requirement for children. Everyone under the age of 18 must have the consent of their parents/guardians to get a passport and this consent must be witnessed.
The only exception to this is if the child is married.
If your passport has been lost, stolen or damaged, you must:
If you lose two or more passports, it may restrict the period of validity for your next passport and it may not be possible to receive your passport in less than 10 working days.
The fee depends on the type of passport you apply for. Make sure to apply in plenty of time.
Postal Applications or applications submitted at the Embassy:
Fees can be paid by cash in Czech Crowns in the exact amount or by bank transfer from a Czech bank account.
Please insert the passport applicant’s full name and passport application number in the message for payee field in order to enable us to identify the payment.
If you choose to pay by bank transfer please include a copy of the bank transfer counterfoil slip with your application. Please note that the passport application fee is not refundable.
Cash payments are only accepted if passport applicant applies personally at the Embassy; please do not send cash in postal application.
